Second Lieutenant Jacobs was the co-pilot of a B-25 Mitchell medium bomber that took off with a crew of 6 and other B-25's from Mangaldan Airstrip, Philippines on a strike mission against three Japanese destroyers south of Formosa. While over their target the plane either hit the mast of one of the destroyers and crashed into the sea or were hit by anti-aircraft fire and crashed into the sea near a Japanese destroyer. The entire crew was lost. Second Lieutenant Jacobs is memorialized on the Tablets of the Missing in the Manila American Cemetery in Taguig City, Philippines. - This narrative was submitted with the enrollment form.
