John Alfonso. Hostile, died outright, ground casualty, other explosive device. Operation Cumberland. Convoy ambushed on Route 547, 17 kilometers southwest of Huế Citadel, Thừa Thiên Province, Republic of Vietnam.
DD-1330 says death was result of multiple fragmentation wounds and burns to entire body when the vehicle in which he was a passenger detonated a hostile mine.
Command Chronicle of 3rd Motor Transportation Battalion states that death was a result of RPG-2 recoilless anti-tank weapon, and that Lance Corporal Alfonso was manning the vehicle's 50 caliber machine gun.
Operations Log of 1st Battalion, 4th Marine Regiment states the convoy consisted of 2 vehicles, Lance Corporal Alfonso's being the lead vehicle, which was hit by RPG (rocket-propelled grenade) or satchel charge. Truck was loaded with 60 millimeter tubes, ammunition, and fuel.
